At-home HPV test request form

Most people who are eligible for publicly funded healthcare in Aotearoa can get an at-home HPV test from us. You can do the test yourself and send it back in a free return envelope. This test is free for some people.

• If you're eligible for free cervical screening your at-home HPV test will be free

• If you're not eligible for free cervical screening you can get an at-home HPV test for $35*

The following questions will help us check if you’re eligible. Once you submit the form, we'll process your request and check your cervical screening history and eligibility. We'll send you an email if your tests have been ordered. Or we’ll email you to let you know if you need to pay for your test or do a cervical sample in person.
